The Importance of an Emergency and Everyday Preparedness Packet
How often do you find yourself at a medical appointment wishing you had all your important healthcare information with you and in one location? Do you find yourself signing into a patient portal while sitting in the waiting room, racing to complete paperwork before your appointment? Or, wondering whether your vaccinations are up to date?
Perhaps you need your health insurance cards, a current medication list or list of allergies, vaccination records, the office note from a treating specialist, details about previous hospitalizations or a list of your past and present medical history.
These are just a few examples of why every adult, regardless of age or health status, should have an Emergency and Everyday Preparedness Packet. This packet helps you to be prepared, not panicked!
Being prepared is not just about responding to emergencies. It is about making access to healthcare information easier by keeping your important information current, organized, and readily accessible. Having accurate healthcare information at your fingertips can save time, reduce stress, improve communication among healthcare providers, and help ensure you receive the necessary services and/or treatment.
An Emergency and Everyday Preparedness Packet is one of the simplest yet most valuable tools an individual or family can create. This packet is not a one-time project. It is a living document that should be reviewed and updated regularly as medications, healthcare providers, legal documents, diagnoses, treatment plans, or goals of care change.
I recommend reviewing it at least every six months and after any significant life or health event. By keeping it current, you will always have accurate information available for both routine needs and unexpected emergencies.
What Does an Emergency and Everyday Preparedness Packet Include?
Your Emergency and Everyday Preparedness Packet should contain the information healthcare providers, family members, caregivers, and emergency responders need to quickly understand your medical history, legal wishes, treatment plan, support system, and emergency contacts.
The packet should include but is not limited to:
- Current medications, allergies, medical diagnoses, surgical history and hospitalization, and past medical history
- Vaccination records
- Health insurance, Medicare, Medicare Advantage, Medicare Supplement, and prescription plan information
- Advance Directive (Living Will)
- Medical Power of Attorney
- Financial Power of Attorney
- HIPAA Authorization, when applicable
- Guardianship or Conservatorship documents, when applicable
- POLST, MOLST, MOST, or Durable DNR orders, when applicable
- Emergency contacts
- Healthcare providers and specialists
- Preferred pharmacy and hospital
- Current treatment plan and goals of care
- Blood Type
- Home care agencies, agency staffed caregivers, private caregivers, skilled home health agencies, hospice, palliative care providers, adult day programs, transportation services, or an Aging Life Care Manager
- Important household information, utility contacts, entry codes, Wi Fi information, and pet care instructions, when applicable.
Why It Matters Every Day and During Emergencies
Instead of searching through your phone, logging into patient portals, or trying to remember important information, everything is organized and immediately accessible. This allows you and your healthcare providers to focus on your care rather than paperwork.
An Emergency and Everyday Preparedness Packet also helps ensure that your healthcare wishes are understood and respected. An Advance Directive communicates your treatment preferences, while a POLST, MOLST, MOST, or Durable DNR for example is a physician’s medical order that guides emergency medical personnel and other healthcare professionals.
Peace of Mind Starts with Being Prepared
Whether you are managing your own healthcare, assisting a spouse, caring for an aging parent, or supporting another loved one, an Emergency and Everyday Preparedness Packet provides confidence that essential information is immediately available when needed. Being prepared is one of the greatest gifts you can give yourself and those who may one day need to advocate on your behalf.
